The Institute for Health Equity and Innovation is an authorized provider of the Inclusion SCORECARD for Population Health™ (developed by Impact4Health). This is an interactive on-line dashboard of best practices that health systems can use to address health care inequities, organized into four distinct focus areas:
Tracking metrics of inclusion (for patient outcomes, staff and community diversity)
Creating a culture of inclusion to support health equity
Managing the link between health equity and quality outcomes
Fostering inclusive community engagement to address the determinants of health
